Practice pharmacists have a role in helping patients manage chronic diseases like diabetes, osteoporosis, asthma or lung disease, and heart disease. They work with your doctor and your regular community pharmacy to ensure you are getting the best out of your medicines.
They provide a range of service, from a quick session on fine-tuning your asthma inhaler technique, or as complex as a full medication review done in consultation with your doctor to make sure your medicines are the best possible combinations available, and recommend ways to simplify your medicines if at all possible.
