75 + Home Health Assessments

All patients 75 and over are eligible for a Home Health Assessment. These assessments provide an overall heath summary, which identifies any areas of concern with the patient's health and any services they may need to help them cope at home for as long as possible.

These assessments provide other health providers, such as ambulance, hospitals etc. with an up to date health summary on the patients, which helps immensely in treating any unexpected future hospitalisations that may occur.

As mentioned above these assessments are done at home by one of our registered nurses who collect the information required by the doctor to assess whether the patients requires any extra services for health problems or to improve their ability to cope at home.

After the home visit, the patient makes an appointment with their GP for follow up and discuss the outcome of the assessment. Such assessments are bulk billed.
