Chronic Health Assistance Scheme

Chronic disease patients may be entitled to GP Management Plans, Team Care Arrangements through the Enhanced Primary Care Scheme through Medicare.

Patients receive 5 visits in total from the services- Podiatry, Physiotherapy, Dietition, Diabetes Educator, Exercise Physiology. These services are either bulk billed or privately billed with a small gap as most is covered by Medicare rebate.
